This application form is to be filled out by a parent or legal guardian. Filling out the application form means you are serious about applying for a position at The Music Alchemist, Kids Rock or Teen Rock. After completion of the application you shall be informed whether the applicant named has been successful. You shall receive an acceptance letter, where an applicant is only deemed to be fully enrolled and accepted once payment has been received in full.
